Enrollment

  • Admission is open to anyone with a physical, intellectual and/or emotional impairment, regardless of age, race, color, sex, religion or national origin. FAR works with people with a wide array of diagnoses and welcomes all who can benefit from its services.

  • New Clients: To start the enrollment process, please reach out to the FAR office at 248.646.3347 to discuss therapy options.  Finding the right fit for a client at FAR requires careful communication and assessment.

Annual

  • Submission of Signed Enrollment Forms and evaluation fee of $50 annually

Each Semester

  • Client Evaluation (or Progress Report) by Therapist
  • Treatment Plan: Recommendations, Goals and Group Authorization(s) (as appropriate)
  • Submission of Signed Registration Form and Tuition & Materials Fees (as applicable)

Electronic Enrollment Forms:

As of August 2025, all enrollment forms are processed through Ensora, our Electronic Health Records system. Ensora enrollment forms are sent out via email by our office every fiscal year.
